New Program
After traveling in Palestine and Jordan, Working in Falasteen ( just one month) and seeing medical centers close for either lack of supplies, or lack of funding, even as there were able bodied physicians and Techs, and administrators sitting around waiting to do someting.
Living in Puerto Rico and Seeing Haiti fall into near Anarchy time and time again, and watching the Dominican Republic wallow in poverty just across the sea. Even as the Medicines and supplies they would have needed where a 55 mineut boat ride away south accross the Carrebean.
After working in Chicago in underserved medicine, and trying to connect poor and immigrant populations to Health care, that existed, but they didn't know about, or that they knew about but didn't know how to access.
The problem, seems consitently to arise at the top.
People are willing to help, the supplies can be aquired, but for some reason there is a disconect, so clinics close, medically underserved areas remain so, even as the hospital has a surplus of Medicine and Doctors.
Vaccines expire while sitting on shelves in Puerto Rico, while Haiti has no vaccination program.
Why on earth is the system so inneficient?
And how would one go about fixing it?
It's like the United Nations took over world healthcare. They mean well but are powerless and incompetent to do anything.
tehy answer to too many people, too much politics plays too determental a role in healthcare access and administration.
But it comes down to, how do you connect waht is needed to those who need it?
how do you make distribution equitable?
how do you make it feasable?
no one seems to be against healthcare, but networks always seem to be just barely chugging along, and even as they do, they leave huge gaps in important places.
so how does one fix this?
how does one integrate the network?
it seems that this is a problem for an economist, and a computer programer to tackle.
it has nothing to do with actuall medicine, but rather the beaurocracy of health.
any computer programers out there? any economists? any ideas?

it'd be blind stumbling.
hopefully people who can create political will, the press, the leaders, the demagouges, whomever, will listen to the economists, but the economists are the key feature. like a captain in a ship.
I disagree that economists direct political will. Greed directs political will and economists are just a tool out of many used to satisfy the greed of the powerful. IMF is a bunch of economists as you said yet IMF is a tool in the hands of the powerful to rein in and control the weak.
in fact, programs taht seem to work in New york have failed in Chicago.
although, the cease fire intervention seems to be growing into a major multi-city success (i've worked with them! peripherally only, but still! mashallah they show so much promise!)
the seatle HIV/AIDS system doens't work too well in chicago either, but we are trying. it's a difference based on how the communities are set up, i don't understand it mysefl, but homegrown programs seem much more tailored ot their individual situations and work the better with the local eccentricities and political/social quirks of the town.
i have no doubt that a drug rehab program made in DC would be an utter failure in Puerto Rico.
gotta make it home grown.
The system has t obe designed so people know whats where and when they can exect to get it, it needs to have a strong predictive component to understand what problems will crop up, (aging populations? birth rates? ages of children and vaccination needs... etc)
the IMF are a bunch of economists.
they run the show you're right, the differnece is in countries that had their own economics plans (most of the Asian tigers) and planned hteir own routes rather htan following IMF doctrine, it turned out much better.
the economists are needed because these countries don't have thier own "intellectual base" there that understnads the particulars of each individual nation.
the economists will set thigns to work, and the money will pay the programers, and the peiple who will buidl and sustain the networks.
even now, in chicago, we loose (yes loose) patients inthe psychiatirc system.
programmers can fix that.
the networks iwll run more efficiantly with efficient programs .
We live in a world where the strong and powerful dominate. Look at the IMF how loans are given out and under what conditions programs are developed to help third world countries and some growing economies such as Brazil and Argentine. Look at "how" the foreign aid is given out by the US and other Euro countries and Japan. Aid is supposed to Aid programs such as health care and so forth right?
Your only hope is a new program here in the US and only the US. Don't try to venture to Palestine, Puerto Rico or Haiti. Do you get me?
I got into the wrong field, i just realized this over the summer, Physicians and people who work in public health have an auro of being the good guys, but they are foot solders in this situation, and we need generals and tacticians.
the doctors who do the best work, who are the msot helpful, are the ones who do it by not practicing medicine.
what an unfortunate thing to learn after 3 years in medical school...
but, better now, than after 30 years of medical practice i guess.
I don't believe this problem is caused or can be solved by an economist or a comp scientest. This subject is one which I often come across helping grad students with their research papers and much to my surprise there are many just like you and me ask the same questions and feel for the poor and unfortunates and this indeed reassures me that there are still good ppl out there from all walks of lives, religions and ethnicties.
very interesting subject, I'll write more later and share my opinion. I'll aslo ask my collegues here at Columbia and share their feedback. I must tell you I'm very much impressed with the subjects you post.
Later my friend
