Observership is just observing without hands-on expereince. In electives, you may be allowed direct patient contact, but you generally have to be still in medical school to get that. Externship and observerships are sometimes used interchangably.

Anything that gives you direct contact with the patients, be it history taking, physical exam, entering notes, are always superior to mere observation.
